If you are receiving a Teaching Assistantship (GTA) or Research 
Assistantship (GRA) this semester,  and need to maintain full-time 
status for health insurance or  student visa status,  please send me 
this form  - http://registrar.gmu.edu/forms/GAF.pdf   You'll fill in the 
top area and sign, and I will fill in the department verification area.  
I will then send it to the Registrar for processing.

If are taking one credit of dissertation or thesis research (999 or 799) 
and need to maintain full-time status for health insurance or student 
visa status, please fill out and send me the form 
http://registrar.gmu.edu/forms/DDR.pdf  if you are a doctoral student
or http://registrar.gmu.edu/forms/MTR.pdf for Master's students
