Just FYI.  The MS in Biostatistics has been reduced to 30 credits effective Fall 2017.  If you wish to change from the MS in Statistical Science to the MS in Biostatistics program, the appropriate form is:

However, you cannot change programs if Fall 2017 is your first semester; in that case, you will need to wait until Spring 2018 to change your program
