I'm going to answer in general terms rather than the specific MarcEdit procedure.  In the above example, you have two ways of telling which field you want to add the prefix to.  First, it has indicators of 40 instead of 41 or 42.  So you could simply replace "40$u" with "40$u" followed by your prefix.  If you have some records with multiple 856 40 fields and only want to add the prefix to one of them, then you could replace "40$uhttp://dx.doi.org" with "40$uYOURPREFIXHEREhttp://dx.doi.org".  That's assuming that all the fields you want to replace have dx.doi.org, of course.

Sure, just delete the ones with indicators 41 or 42.  If you also have other fields with 856 40 that you want to get rid of, you could change the one you want to keep (e.g. containing "dx.doi.org") into a 956, delete all remaining 856, then move the 956 back to 856.
